Application of native T1 map in characterization of acute myocardial infarction: can T1 distinguish between infarct area and area-at-risk?

چکیده
Background Combined assessment of edema and necrosis using conventional T2-weighted sequences and late gadolinium enhanced (LGE) imaging has been established as the reference standard for in-vivo assessment of myocardial damage in acute myocardial infarction (AMI). However the standard CMR protocol is time-consuming, not tolerated by patients in poor clinical conditions and needs of gadolinium administration. Moreover, T2 weighted images are not always of good image quality and are more prone to be affected by artifacts. Our purpose was to investigate the capability of native T1-mapping to differentiate with a

Pre-Contrast Imaging of the Area At Risk in Acute Myocardial Infarction: T2 yields higher contrast than T1 MRI
Introduction: The area at risk (AAR) of infarction is the hypoperfused myocardial bed during coronary occlusion. The edematous AAR can be delineated in-vivo by T2 MRI and can be used in conjunction with delayed contrast enhancement to calculate myocardial salvage. T1 MRI has been recently used to image the edema associated with acute myocardial infarction. Objectives: We hypothesized that, due ...
متن کاملQuantification of both the area-at-risk and acute myocardial infarct size in ST-segment elevation myocardial infarction using T1-mapping
BACKGROUND A comprehensive cardiovascular magnetic resonance (CMR) in reperfused ST-segment myocardial infarction (STEMI) patients can be challenging to perform and can be time-consuming. We aimed to investigate whether native T1-mapping can accurately delineate the edema-based area-at-risk (AAR) and post-contrast T1-mapping and synthetic late gadolinium (LGE) images can quantify MI size at 1.5...
متن کاملAcute Infarct Extracellular Volume Mapping to Quantify Myocardial Area at Risk and Chronic Infarct Size on Cardiovascular Magnetic Resonance Imaging.
BACKGROUND Late gadolinium enhancement (LGE) imaging overestimates acute infarct size. The main aim of this study was to investigate whether acute extracellular volume (ECV) maps can reliably quantify myocardial area at risk (AAR) and final infarct size (IS). METHODS AND RESULTS Fifty patients underwent cardiovascular magnetic resonance imaging acutely (24-72 hours) and at convalescence (3 mo...
متن کاملTiming of post contrast T1 values affects the extracellular volume fraction in ST segment elevation myocardial infarction
Background Native T1 is increased in acute myocardial infarction. Extracellular volume fraction (ECV) can be calculated using T1 mapping following contrast. The timing of imaging following contrast in acute myocardial infarction is not established. 15 minutes has been proposed for conditions with and ECV < 0.4. 15 minutes may not be applicable in acute myocardial infarction due to infarct chara...
متن کاملPrognostic significance of infarct core pathology revealed by quantitative non-contrast in comparison with contrast cardiac magnetic resonance imaging in reperfused ST-elevation myocardial infarction survivors
AIMS To assess the prognostic significance of infarct core tissue characteristics using cardiac magnetic resonance (CMR) imaging in survivors of acute ST-elevation myocardial infarction (STEMI). METHODS AND RESULTS We performed an observational prospective single centre cohort study in 300 reperfused STEMI patients (mean ± SD age 59 ± 12 years, 74% male) who underwent CMR 2 days and 6 months ...
